Eden Hazard has hailed the impact made by Antonio Conte after the Italian led Chelsea to their fifth Premier League title in his first season at the club.

A late goal from substitute Michy Batshuayi, who had only been on the pitch for six minutes after replacing Hazard, secured the Blues fifth championship of the Premier League era – and a second for the Belgian international.

"It's amazing to celebrate with the fans,” said Hazard at full time. “The game was so difficult. We are very happy.

"We talked a lot about the manager already, he is fantastic and we work a lot in training. Full credit to him and the players. We lost to Arsenal and then we went on an unbelievable run.

How Antonio Conte turned Chelsea into title winners

"That's when we said that we can win the league."
